There have been few advances in the treatment of difficult headaches in many years. Oral medications are poor choices for frequent and severe headache. Physical therapies such as massage and postural training are worthwhile but often ineffective.Â
Injection treatment with a medicine that relaxes muscle and blocks pain nerve fibres is often helpful and can be administered in minutes. It has an excellent safety profile and lasts some months after one injection session.Â

Prolotherapy is the injection of a strong glucose solution into ligamentous tissue. This has the effect of stimulating collagen repair in injured tissues. The treatment has many applications such as assisting joint repair in arthritis, and in tendonitis and bursitis management.
Injured muscle tissue develops trigger points – localised areas of retained muscle spasm which cause pain and muscle dysfunction. Injury can be acute such as in a sporting ‘strain’ or chronic such as the ‘knots’ in postural muscles of shoulders and hips.
Trigger point injections were initially based on traditional acupuncture where ‘ah shi’ points coincide with trigger points. Trigger point injections use local anaesthetic to improve the outcome of needling.Â
The most common points of injection in headache problems are the upper shoulder areas within the trapezius muscles.Â
